Me: From your browser the form could be customized to have more textboxes, checkboxes, etc. and you can drag them to whatever size or position you want them in. Once a user submits a form, it gets tracked in the database, where you can also view their data in the reports page. You can create a form for anything, ask as much as you want, and access it from anywhere.
